Reaching the
Tipping Point
The goal of the second John A. Hartford Foundation Tipping Point Challenge is to catalyze innovation that will make positive, breakthrough change in the care of serious illness.
We need this change to take place within every organization and throughout the nation.
Three Categories of Innovation
The Tipping Point Challenge is open to all health care organizations and collaborations, in any setting, discipline, or specialty, anywhere in the US. You do not have to be a CAPC member organization. The competition is focused on innovation and takes place in stages over a 10-month period. View the guidelines.
Specifically, we are looking for submissions of initiatives that address any of three categories of innovation.
Building Skills Among Nonpalliative Care Specialties and Disciplines
Improving communication skills and/or pain and symptom management skills among all specialists treating CHF, COPD, and cancer
Access to Palliative Care
Improving systematic access to specialty palliative care
Reaching the Patients in Need
Implementing a population health approach to identify and address patients with serious illness and unmet needs
Please note: The COVID-19 crisis is testing clinical teams with an unprecedented number of patients, rapidly changing policies and procedures, and shifts in roles and priorities. Teams are welcome to submit initiatives they have introduced during the pandemic.
HOW TO PARTICIPATE
STEP ONE
Sign Up: June 2020-March 2021
Any organization may participate. You do not have to be a CAPC member. Multiple organizations may collaborate on a single initiative. You may also sign up in two or all three areas of innovation.
STEP TWO
Full Project Submission: April 2021
Describe the initiative, the innovation, the bold solutions and the outcomes. The full submission must address the following: impact, evidence-base, feasibility, scalability, and sustainability.
STEP THREE
Review and Selection of Top Twenty-Five: May-June 2021
Top Twenty-Five: Announced July 2021
Using a common rubric, the Advisory Team will assess the criteria outlined above. Based on the scores, the Top Twenty-Five will be announced (July 2021).
STEP FOUR
Winners Announced at the CAPC National Seminar: November 2021
In each area of innovation, three winners – Gold, Silver, and Bronze – will be announced at the CAPC National Seminar (November 2021).

EVENTS
CAPC hosts member and open-to-all events throughout the year. We encourage you take full advantage, as these events provide technical assistance, build clinical and operational skill, share information and best practices, convene the field, improve access and awareness, and promote the ideas that grow out of the competition.
